In-Home Services

CoAction’s Home and Community-Based Services team works together to help people. They coordinate different services to meet individual needs. Care Managers partner with individuals to create special plans that focus on what each person wants. These plans help people live independently and get care at home. The team determines the best combination of services, like delivering meals, having someone take care of them, or using emergency devices. It’s like having a team of helpers who make life better!

  • If you need to reschedule your services or discuss issues, please contact the agency that provides your care.
  • If you are not able to resolve an issue with your provider or need other assistance, please contact your CoAction Care Manager.
  • Please inform your Care Manager if you are admitted to the hospital or if you are not being properly served by your In-Home Service Provider.
  • Your Care Manager can help you create a back-up plan so that you have care in the unlikely event that services are delayed or cannot be provided.
  • Contact the Care Manager Supervisor if you feel your Care Manager is not working with you and meeting your needs.
